MANAGED CARE PROGRAMS
Sycamores’ Enhanced Care Management (ECM) program supports members in the program who are struggling to access health care and social resources. We work side by side with members to ensure they get the right care at the right time, in the right setting to live healthier lives.
Because housing is key to good health, our Community Support (CS) team provides individualized housing navigation support to help members find, fund and sustain affordable housing.
